End-of-Life Care touches people of all ages and includes both the individual and their family. It engages all parts of the health care system; from hospitals, hospices and long-term care facilities, to communities, and the home environment. End-of-Life Care addresses the physical needs of a person but also the spiritual, emotional mental health and financial needs of the individual and the family.
